Findings
Alleged Victim (AV) was care planned not to leave the facility without supervision. On or about December 11, 2021, AV left the facility without supervision and fell resulting in a fracture and head injury. The facility failed to follow the care plan which is a violation of resident rights, is neglect of care and constitutes abuse.
