Findings
On or about December 12, 2021, Alleged Victim (AV) was in the facility's dining room and fell. Based on facility documentation and interviews, it was determined that the tables and chairs were close together causing AV to trip and fall as he/she was ambulating in the dining room. The facility's failure to provide a safe environment resulted in AV experiencing a fall, was transferred to the hospital for treatment and diagnosed with a hip fracture which is a violation of resident rights, is considered neglect of care and constitutes abuse.
