Findings: The Alleged Victim (AV) is a known fall risk and suffered multiple falls with and without injury. AV was care planned to have safety checks, however, there are no safety checks noted for the dates that AV had fallen. AV had fallen on December 13, 2021 resulting in back and neck pain, and fell again on December 14, 2021, resulting in a laceration and a goose egg to his/her head. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00160 $450.00 fine assessed
