Findings
On or about December 12, 2021, Alleged Perpetrator 2 (AP2) stuffed rubber or latex gloves into Alleged Victim's (AV) mouth while assisting AV with his/her Activities of Daily Living. AP2's actions is considered physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
