- Witness 1 (W1) relies on the facility for his/her care. W1 has a history of agitation, behavioral disturbance, and resident to resident altercations. On or about December 26, 2021, W1 behavior escalated from verbal incident to physical incident towards Alleged Victim (AV). W1 slapped AV on the right cheek. The facility failed to properly plan care and implement interventions for W1 behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
