Findings
The facility failed to provide assistance with bathing to the Alleged Victim (AV), who was not put on the shower schedule. Resulting in AV going without a shower for over a month and experiencing unreasonable discomfort and a serious loss of dignity. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
