- The Alleged Victim (AV) and Witness #1 (W1) are care planned to be kept separate from each other as they have a history of aggression. On or about January 8, 2022, AV and W1 passed by each other, W1 hit AV in the face with his/her shoe. AV was not injured, however, AV was placed at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
