Findings
Alleged Victim (AV) has a known history of falls. Between October 24, 2021, through December 24, 2022, AV suffered four (4) falls in his/her room, some resulting in injuries. On or about January 18, 2022, AV fell in his/her room, suffering an abrasion to his/her right leg. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
