Findings
The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, the facility failed to implement a service plan and failed to complete a change of condition. AV experienced significant skin injuries and went without medications, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
