- Alleged Victim (AV) relies on facility for his/her care. AV and Witness 1 (W1) have a history of resident-to-resident altercations. On or about January 25, 2022, AV and W1 got into an altercation where AV hit W1 with a doll. The facility failed to appropriately care plan for AV and W1's known behaviors, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
