Findings
The facility failed to provide appropriate health assessment, oversight, and monitoring when AV’s medical condition changed which resulted in AV experiencing a decline in his/her condition, resulting in AV not receiving timely medical treatment. AV was admitted to the hospital on or about, January 19, 2022, where he/she spent time in the hospital receiving treatment for a serious infection. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
