Findings
According to the documentation, the facility failed to follow the Alleged Victim’s (AV) service plan resulting in repeated falls. The failure resulted in AV experiencing injuries including bruising and bumps on the head, causing pain and unreasonable discomfort. The facility failures are a violation of resident rights, are considered neglect of care and constitute abuse.
