Findings
The facility failed to provide appropriate services and properly care plan according to AV’s needs and behaviors. The failure resulted in AV experiencing repeated unreasonable discomfort, loss of personal dignity and worsening of his/her skin condition, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
