Findings
The facility failed to provide appropriate services related to AV’s need for assistance with ambulating, toileting and catheter care. The failure resulted in AV experiencing a loss of personal dignity and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
