- The Alleged Victim (AV) had a history of wandering and checking for open doors. On or about February 4, 2022, AV was brought back to the assisted living side of the facility by a good Samaritan that saw AV at a nearby mall and assumed AV was from the facility. Staff in the memory care checked all doors and found that even though locked, if pushed hard enough would open. AV left the facility without assistance, placing AV at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
