Findings
On multiple occasions, it has been noted by the Alleged Victim's (AV) private caregiver that AV's catheter bag was not emptied during the evening hours, causing AV's catheter to back up and leak, leaving AV and his/her bed soaked in urine. AV was placed at risk for harm and unreasonable discomfort by the facility staff not emptying AV's catheter in a timely manner. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
