Findings
The Alleged Victim (AV) relies on the facility for his or her safety. In the evening, on or about February 8, 2022, AV was found at a nearby grocery store approximately half a mile away from the facility. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
