Findings
The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV had fallen from his/her wheelchair on January 23,2022 and no interventions were put into place to ensure resident safety. On or about February 12, 2022, AV suffered another fall from his/her wheelchair, resulting in a cut and swelling to his/her hand and bruising and swelling of his/her left eye. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
