- The facility failed to implement interventions and appropriately care plan related to Alleged Victim's (AV) fall history when AV was diagnosed with a communicable disease necessitating quarantining AV in his/her room. The facility's failure resulted in AV experiencing two falls in the same day in which one of the falls required AV be transferred to the hospital for emergency evaluation in which AV was diagnosed with a fracture to his/her T6 vertebrae requiring surgery. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
