Findings
The facility and Alleged Perpetrator #2 (AP2) failed to provide a safe environment. On or about February 10, 2022, AP2 entered the community lounge where the Alleged Victim (AV) was. In an attempt to be playful, AP2 shot a nerf gun at AV, hitting him/her in the eye, causing injury to AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facilities failure to provide a safe environment is a violation of Oregon Administrative Rules.
