Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1 (W1). An incident occurred between W1 and the Alleged Victim (AV) where they were each gripping and squeezing each others arms. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
