Findings
Alleged Victim (AV) has a history of bruising and skin tears due to his/her removing protective sleeves on his/her arms and pads on his/her wheelchair. AV has been found with injuries on his/her lower arm and/or back of his/her legs on or about February 4th, 19th and 22nd, 2022, and no new interventions have been implemented to prevent new injuries. The facility failed to care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
