Findings
The facility failed to provide appropriate supervision when Alleged Victim’s (AV) participated in a facility activity event that involved a beach ball. At time of incident, AV was standing, observing the beach ball activity when the beach ball came near AV and he/she hit the beach ball and fell. AV was sent to the hospital and was diagnosed with a broken hip. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
