- The Alleged Victim (AV) ran out of pain medication before he/she was supposed to. AV was prescribed two pain pills per day. On or about February 6, 2022, AV was administered 24 tablets and again on February 8, 2022, AV was administered 16 tablets those days. Alleged Perpetrator #2 (AP2) was the staff member that signed out those medications. A three week investigation determined that AP2 is the person that took the medication from AV. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial exploitation. The facility's failure is a violation of Oregon Administrative Rules.
