Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about March 22, 2022, it was discovered that AV was being repeatedly left in his/her wheelchair at night and had developed a pressure ulcer. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
