Findings
The facility failed to implement interventions and appropriately care plan related to Witness 1’s (W1) known behaviors. On or about February 21, 2022, W1 was found inappropriately touching the Alleged Victim (AV) causing a loss of personal dignity, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
