Findings
On or about January 4, 2022, the facility discovered the Alleged Victim (AV) had one (1) full controlled medication card missing. There had been no administration from the prescription, explanation of destruction or evidence of discontinuance. Facility was not completing narcotic count or narcotic disposal per Facility policy. There is no direct evidence indicating that Alleged Perpetrator 2 (AP2) took the Oxycodone. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
