Findings
According to the documentation, the facility failed to follow the care plan to supervise the Alleged Victim (AV) while ambulating and ensure the use of his/her mobility device. The failure resulted in the AV experiencing a fall in the courtyard, receiving a laceration to their forehead requiring medical evaluation, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
