Findings
The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her change of condition, number of falls with lack of appropriate interventions. AV has a history of falls without appropriate interventions, indicated in case 00183255. On or about February 10, 2022, AV had another fall which resulted in AV experiencing unreasonable pain and discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
