Findings
The Alleged Victim (AV) relies on the facility for his or her safety. On or about March 5, 2022 the facility received a telephone call that AV was outside the locked facility. The facility is unsure how AV was able to leave. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
