Findings
On or about March 12, 2022, Witness 1 (W1) grabbed a knife during an argument with Alleged Victim (AV). W1 is known for having both physical and verbal aggressive behavior which leads to altercations with other residents. The facility failed to provide a safe environment for AV which put AV at risk of significant harm and caused AV emotional harm. The facility's failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
