Findings
On or about March 11, 2022, Alleged Perpetrator #2 (AP2) went to the Alleged Victim's (AV) room to assist with a shower. AV did not want a shower, and balled up his/her fists at AP2. AP2 yelled and cussed at AV and grabbed ahold of AV. AV was very upset and continued with behaviors after the incident. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes verbal abuse. The facility's failure is a violation of Oregon Administrative Rules.
