Findings
The facility and Alleged Perpetrator #2 (AP2) failed to put interventions in place to address Alleged Victims (AVs) significant change in condition and failed to provide services to care for AVs pressure ulcers. AV experienced pain and unreasonable discomfort. AP2’s actions are a violation of resident rights, are considered neglect of care and constitutes abuse.
