Findings
The facility failed to appropriately care plan for the alleged victim (AV) regarding his/her risk of falls. AV was found on the floor from a fall at least seven times between January 1st, 2022 and March 20th, 2022, no new interventions were put in place to mitigate AV’s fall risk. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
