Findings
Alleged Victim (AV) relies on the facility for his/her care. AV has multiple small bruises on his/her left shoulder. When turning residents, staff hold onto their shoulders. Staff are not adequately trained on how to turn or transfer a resident. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
