Findings
The Alleged Victim (AV) was known to experience weight loss and need assistance with eating. AV's care plan did not indicate that AV needed assistance with eating or require staff to monitor AV's weight. AV lost a significant amount of weight between January 6, 2021 and March 2, 2021. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
