Findings
The facility and the Alleged Perpetrator 2 (AP2) failed to follow AV’s care plan to be a one person assist during ambulation. On or about October 27th, 2021 the AV fell while unsupervised during a shower transfer, AV was transported to the hospital and diagnosed with a hip fracture. The facility and AP2's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
