Findings
The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her change of condition, increased level of care and number of falls with lack of appropriate interventions. On or about March 29, 2022, AV was transported to the hospital as he/she could not bear weight on one leg as a result of a fall the previous day. At the hospital, AV was diagnosed with a fractured tibia, low potassium levels, and a urinary tract infection. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
