- On or about April 1, 2022, it was discovered that the Alleged Victim (AV) was missing pain pills from his/her prescription. Alleged Perpetrator #2 (AP2) counted AV's medication and reported that 15 pills were missing. AP2 is seen on camera counting the medication, then leaving the camera view with the medication, and can be heard opening the bottle and pills moving. AV self-administers his/her medication and it is not facility policy to count self-administer medications. AP2 later tested positive for the pain medication. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes financial abuse. The facilities failure to protect AV from financial abuse is a violation of Oregon Administrative Rules.
