Findings
On or about March 23, 2022, Alleged Victim had an unwitnessed fall and was found on the floor with h/h dresser and television on h/h. AV sustained laceration to face and abrasion to right knee. The facility failed to maintain a safe and hazard-free environment, which is violation of resident’s rights, is considered neglect of care and constitutes abuse.
