Findings
The facility failed to provide a safe environment by not providing appropriate supervision and staff support regarding known behaviors between the Alleged Victim (AV) and Witness 1 (W1). W1 threw coffee on AV approximately three times, in about one week. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
