Findings
Witness #1 (W1) is known for hitting other residents when he/she is agitated and he/she is care planned for staff to intervene and offer redirection. On or about April 14, 2022, W1 was displaying behaviors and entered the Alleged Victim's (AV) room and started grabbing his/her belongings. W1 swung at AV with a comb, AV sustained a skin tear to his/her forearm. The facility's failure to intervene is a violation of resident rights, is considered neglect of care and constitutes abuse.
