Findings
Alleged Victim (AV) is care planned to receive one-person assist for all mobility and transfers. AV often attempts to transfer and ambulate without assistance. AV fell on April 4th, 5th, 6th, 11thand 17th, 2022, while unsupervised by staff. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
