Findings
The facility failed to follow both Alleged Victim’s (AV) and Witness #1's (W1) care plan to be monitored at all times in common areas and redirected due to behaviors. Between April 14, 2022, and April 28, 2022, there were at least three (3) altercations between W1 and AV resulting in AV falling. The falls resulted in AV’s repeated unreasonable discomfort and on at least one occasion facial bruising. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
