- On or about March 10, 2022, Alleged Victim (AV) discovered $300 was missing from his/her wallet. Based on interviews and facility documentation, it was determined that AP2 financially exploited AV by wrongfully taking funds belonging to AV which resulted in financial loss to AV. AP2's actions is considered financial exploitation which constitutes abuse. The facility failed to protect AV from theft which is a violation of Oregon Administrative Rules.
