Findings
According to the documentation, the facility failed to follow the Alleged Victim’s (AV) service plan when expressing behaviors. The failure resulted in multiple staff engaging the AV which caused increased agitation and combativeness and suffered and injury fall, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
