Findings
On or about April 21, 2022, Alleged Perpetrator 2 (AP2) failed to review the Alleged Victim's (AV) care plan, and updates. AP2 worked and cared for AV without reviewing the care plan. AP2's actions are considered neglect of care and constitute abuse. The facility failure is a violation of Oregon Administrative Rules.
