Findings
The Alleged Victim (AV) is a known fall risk and was on wellness checks 4 times per shift. AV fell in his/her room, suffering cuts to his/her eye and nose, as well as bruising to his/her eye. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
