Findings
The Alleged Victim (AV) is known to wander and is care planned to be supervised and redirected. The facility failed to follow the care plan, which allowed the Alleged Victim (AV) to leave the facility twice in two days, which resulted in a skin injury. The facility’s failure is a violation of resident rights, is neglect of care which constitutes abuse.
