Findings
On or about January 9, 2022, Alleged Victim (AV) ate his/her cigarette butt and reported that he/she had poisoned himself/herself and asked facility staff to call 911. Facility is responsible to AV's medical care and expected to take reasonable action against poisoning. Based on interviews and facility documentation, the facility failed to provide AV with a safe environment by their inaction in providing essential medical services. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
